Here at Living Waters we have many different services that we offer. One of those being a hospice company! Local to us in Genesee County is Swan Hospice. They have provided us with hospice services and support this last year and have been wonderful!

Here at Living Waters we are not a hospice company but we utilize hospice companies like Swan Hospice to provide our residents with services. Hospice is such an added bonus and extra support when the residents can really use it.


Swan Hospice Goals

"Swan Hospice offers comfort and warmth at a time when it is most needed. As a team, we help patients and families find calm and healing so they can gracefully embrace the impending loss of a loved one. Our staff is competent and trustworthy, but above all, compassionate.

We see our role as a privilege and serve our patients with passion and pride. In addition to the experience we provide and the relationships we cultivate, our hospice care is defined by the permanent impressions we leave on our patients and their families. We believe a patient's end-of-life transition is a milestone, and strive to make the journey a memorable experience."


Services

We provide a tailored hospice care plan in collaboration with a team of professionals to address the needs of each individual patient. The hospice team consists of a physician, medical director, nurse case manager, social worker, chaplain, home health aides and trained volunteers.


Emotional and Spiritual Support

Social workers and chaplains connect patient families with available resources and offer end-of-life counseling, planning, and bereavement services.

Nurse Case Management

A case manager assesses the patient’s needs and provides support to the family. The nurse can offer education on the disease process, and is available for visits and supportive phone calls.

Home Health Aides

Compassionate and skilled, our aides provide companionship and assistance with daily living. They help with dressing, meal preparation, grooming, and more.

Medical Management

An experienced physician directs the hospice team by overseeing the patient’s medical care, recommending medications, and acting as the patient’s attending physician, if needed.

Medical Equipment and Supplies

Our team manages all medications related to the hospice diagnosis. We also arrange for necessary equipment such as a hospital bed, wheelchair, oxygen, and incontinence or wound care supplies.

Volunteer Services

Our network of passionate and committed volunteers provide patients with companionship or skilled support, such as reiki, massage therapy, pet therapy, and music therapy.


While we do think that utilizing our facility preferred companies is a good, safe and convenient decision we also respect and honor the wishes of each resident. If you or your loved one prefers to stick with your current hospice provider we understand that completely. This is just an AWESOME option for you while you live here.
